  3. Date of birth
  4. Telephone number
  5. Social Security Number
  6. Income information
  7. Mother’s maiden name
  8. Credit or debit card numbers
  9. Other purposes as permitted under applicable law

How will we use your information?

We will use only the information that we need to perform authorized functions  or provide other services as permitted under applicable law. The following are some of the functions that we may perform and services that we may provide:

  1. Providing you with information about your health insurance coverage options and insurance affordability programs for which you may be eligible.
  2. Helping with your application for insurance. Answering questions about your eligibility.
  3. Helping to enroll you in a qualified health plan.
  4. Helping with filing appeals of eligibility determinations.
  5. Correcting errors in your application.
  6. Other services as permitted under applicable law.

Will we share your information with anyone?

We may only share your information as described in this notice. We may share your information with your authorized or legal representative, certain federal or state agencies, health insurance issuers, or subcontractors that help us to provide services to you. We must receive your permission to share your information for any purpose that is not described in this notice.

Is the request to collect your information mandatory or voluntary?

Your decision to provide your information is voluntary. We need to obtain your informed consent for any use or disclosure of information that is not permissible within the scope of this Privacy Notice Statement and any relevant agreements that were in effect as of the time the PII was collected.

What happens if you decline to provide your information with us?

You have the right to refuse to provide PII, or to limit the PII you provide, to us. However, if you refuse to provide your PII, we may be unable to successfully enroll you in a health plan or otherwise assist in facilitating your receipt of Advance Premium Tax Credits or Cost-Sharing Reductions. In such case, we may attempt to refer you or your legal or authorized representative to another person or entity who may be able to help you.

What is the legal authority to collect PII?

Federal laws and regulations permit us to collect PII for various reasons. Federal laws and regulations include Section 1411(g) of the Patient Protection and Affordable Care Act (42 U.S.C. 18081(g) and 45 C.F.R. 155.260) and 45 CFR 155.220.
