If you are enrolled in Original Medicare, you may sign up for Medicare Part D as well, which is overseen by CMS, but sold by private insurance carriers. Your monthly premium is based on the plan that you select, but can also be adjusted by your income just as Medicare Part B can be adjusted by premium. Or, you can enroll in a Medicare Advantage Plan (Part C) that will typically include drug coverage. Medicare Advantage plans that include Part D prescription drug coverage will also include adjustments based on income.
